About the Role
The Designated Coordinator plays a key role in ensuring high-quality service delivery for clients receiving 245D services. You'll provide oversight, coaching, and support to direct care staff, monitor client progress, ensure compliance, and act as a trusted advocate for the people we serve.
This is an in-office position (Bloomington), with the ability to travel to client homes a couple times per week (within the Twin Cities Metro). Mileage reimbursement is offered or client home visits.
What You'll Do
- Oversee, support, and evaluate service delivery activities.
- Help clients achieve outcomes outlined in their Support Plans.
- Observe and assess direct support staff competency.
- Evaluate service methods and the effectiveness of service delivery.
- Maintain compliance with all 245D licensing requirements.
- Communicate with case managers, counties, and internal teams.
- Maintain accurate documentation, reports, and client records.
- Complete and review incident reports, corrective actions, and emergency procedures.
- Visit clients in their homes as required.
What You Bring
Education/Experience - must meet one of the following:
- Bachelor's degree in human services/education/health + 1 year of full-time work experience providing direct care services to people with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children; OR
- Associate degree in human services/education/health + 2 years relevant experience (see above); OR
- Diploma in human services/education/health + 3 years relevant experience (see above)
Preferred qualifications:
- Experience in a 245D setting
- Experience working in a home healthcare setting or case management
- Strong organizational and communication skills
